For 5 Additional Clock Hours Mail Photos To get 5 additional Clock Hours of Continuing Education do Learning Activities 2, 5C, and 6, described below, which involve the taking of photos, for a total of 15 Hours. However all three Learning Activities 2, 5C, and 6 must have photos to receive the 5 additional optional hours of credit. Partial credit cannot be awarded. Therefore all photos as described in Learning Activities 2, 5C, and 6 must be provided to receive the 5 additional Clock Hours of Continuing Education.
Should you not wish to do Optional Learning Activities 2, 5C, and 6, and complete the other activities, outlined in the Manual and this Answer Booklet, you will receive 10 hours of NCCAP Pre-approved CE Credit. The choice to do the activities involving photos and receiving the added 5 CE hours, for a total of 15 hours of pre-approved NCCAP CE credit; OR to not do the photos, described in Learning Activities 2, 5C, and 6 below, and receive 10 NCCAP Pre-approved CE hours...is up to you.
How to Mail Photos for the 5 additional Clock Hours Should you wish the 5 additional Clock Hours of credit described above, the photos in Learning Activities 2, 5C, and 6 can be submitted by printing a hard copy of this completed Answer Booklet. Mail completed Answer Booklet, with photos affixed, via Regular First Class mail to the Healthcare Training Institute, PO Box 6566, Bloomington, Indiana, 47407. Note, Certified, Registered, or Express Mail will cause processing delays. Due to the photo requirement to get the 5 additional hours: 1. certificate cannot be issued on line; 2. Answers to the 17 questions above must accompany your photos; 3. Separate certificates will not be issued. Learning Activity #2 - Construction of Success Therapy® Projects Construct projects described in the Manual you feel are appropriate for your resident population. Take photos of projects and place photo below. This Photo is not needed to receive 10 CE’s, but must be attached to receive 5 additional CE’s.

Learning Activity #6 - Success Therapy® Bags in the Resident’s Room This set of photos does not need to show residents, but may. The bag contains the one Success Therapy project the resident is using, along with the one corresponding Inservice Training Sheet inside found at the back of the Manual. The back of your Manual contains reproducible masters for labels to be affixed to bags and Inservice Training Sheets.
